Disease management programme for heart failure

The programme seeks to improve quality of life for patients with heart failure in the East Coast Area, through the provision of expert multidisciplinary advice, education and support, from diagnosis of condition to death.

The programme provides multidisciplinary heart failure care to ensure patients are on maximum tolerated evidence-based medical therapy and device therapy according to individual patient needs. It also seeks to empower patients to embrace self-care principles of heart failure management with strong support services for patients and family to respond to the deterioration of physical and emotional well being. An important role of the team is to communicate with other healthcare providers, incorporating primary and secondary care, relating to the changing needs of the patients as the disease progresses or improves.

The service currently has two consultant cardiologists, two advanced nurse practitioners, three whole time equivalent clinical nurse specialists, a psychotherapist, physiotherapist, dietician and administrative staff.
